Chasity R. Evans DNP, CPNP-PC is a board certified pediatric nurse practitioner. She earned her Masters in Nursing from Indiana University and her Doctor of Nursing Practice from Frontier Nursing University. She has 17 years of pediatric experience, including positions at Primetime Pediatrics, Riley Hospital for Children, and most recently at Open Door Health Services. She is an active member of the National Association of Pediatric Nurse Practitioners and Sigma Theta Tau Honor Society. Special interests include health and wellness promotion and asthma management. Chasity is a Muncie native and enjoys spending time with her husband Adam and their four children, Madeline, Gracie, Hayden, & Parker.
